Description
I'd like to make this Word doc Pool Safety Evaluation form more attractive.
Three things:
1) The boxes/bullets beside the list are intended to be boxes that someone can check off with a pen or pencil.
2) Under (or beside) "Safety Recommendations" there needs to be room (and maybe lines) for someone to write in recommendations.
3) I will not make the contest blind. I like designers seeing my suggestions to other designs and building off of them.

If you use fonts that require a license, confirm with the client they're ok with it. For licensing reasons, it is better to provide your client with information on how to acquire the font rather than providing the actual files.
