Straight Forward Order Form Redesign...

I'm an entreprenuer starting a new business called AutoPitch.com. I have been working on the web since 1996. I have a pretty good grasp of what I want, but I love getting all of the different perspectives and mockups from 99designs.

For anyone looking to buy a new/used car on AutoPitch.com.

I want to take the order form here:

I would like the new Order Form to look like the one on http://www.audible.com:

So if you look at the order form on AutoPitch you will see a set of icons that pops up with some text. I would like that text and icons to just be on the left of the order form (similar to audible.com order form). We can use the same icons I am currently using or we can use new icons. Whatever looks best.

Also, if you open the order form on Autopitch and click on the first form field, you will see a comment box appear to the right of the form fields. I need to keep those in there, so please try to incorporate that into your design so it all fits.
