Nothing like the old letterheads I have added. That is just an idea of my old designs.
I would like it to look a little like the nice blue A4 folder examples added, but more relevant to shoulder surgery.
The type of folder I need is one with a sleeve to hold pages as shown in one of the images!

What to avoid
Do not use a logo based on my initials like in the example! I may get a LOGO design in the future. Preferably keep the name fairly simple, unless you have a great idea of a small change to the name that would look OK. In that case give two examples - one using the name change idea and second design using a plain name.
