Dental Office Sign on Building

Family Dentist Office
What's your vision?
Need a street-facing sign for the side of a building for a family dental office. Want the sign to be contemporary, but classic enough that it won't look too outdated in 20 years. It also needs to blend with an old tired looking building (attached), so it does not look out of place. The sign will be 3-dimensional, i.e. raised letters and borders. Even though the building looks in rough shape, the sign should be sophisticated and classy-looking, not loud or too busy. It has to strike a balance between making the building look nicer (if that's even possible) without it being so nice or modern that it looks out of place. Shape of the sign, color, additional art up to the designer, but it must contain "Sai Tseng DDS" and "Family Dentist" and be readable from the road. Thank you. Addendum 1: After some initial submissions, I realized I might not be communicating what I want properly. I'd like the equivalent of farmhouse chic, but in a sign (Modern and Classic at the same time if that make any sense.)
I just need a design to submit to the sign maker. I already have someone to make the sign.

And then… they selected a winner!
We were thoroughly impressed with how much work this designer put into our competition. The ideas were creative enough to catch our attention early on, and the designer gave us multiple options to consider. By the time we were choosing finalists, we were fairly sure this designer would win. We just had multiple roads we could go down because of all the options the designer had presented. It was hard to choose! We should also mention that when we gave feedback, this designer was immediately responsive, understood what we were trying to communicate and offered alternatives that matched precisely what we asked for. We really do appreciate the creativity and the level of effort put into our design.
Along the way, they met lots of talented designers…
We think contests are a super fun way to get design.
