The designing process is a two-way street. To get the best layout design, which is most important for both side (client and designer), you should be able to share the next couple of things with me:
1 - Precise design brief - as much you share your thoughts with me about what you would like to see as a result, the more I'll be able to transform that info into a great design.
2 - Clear feedback
3 - Material what you would like to present (photos, text, logo, etc...)

My policy is if you are not satisfied with my work after second (2) revision, you can stop the process and in that case you are not in any kind obligation to me.

My practice is to deliver at the end of the project
1 - raw design file (Adobe Illustrator) to the client and to practically make backup files for both sides,
2 - PDF print-ready version and
3 - PDF for the web or email needs (the same file like print version but much more lighter).
