We are a medical and health physics consulting company providing physics services for medical imaging (Xray, CT, MRI, etc). Our target audience are hospitals and medical imaging clinics.

MS Excel / Word Cover Page for Physics Reports. The design will primarily be used in Excel. The design should include the Client Name, Client Address, Report Name, Equipment Tested, Test Date, Physicist Name, Our Logo, Our Contact Information. The design should be the same for Word and Excel.

We have a current Cover Page (see attachments) that we wanted updated. We also have a new logo that we would like to incorporate. The design should be in Word and Excel. We should be able to edit the text and logos.
