Patient report Folder. In second visit, we will give out a report of findings telling patients about their health conditions. It will include 1) what happened to them 2) How are we going to help them 3) What kind of treatments will they get 4) Duration of the treatments 5) the cost of treatment. So they have better understanding of their health issues. I will need this folder to convey good clinic impression, hope and professionalism to the patients. The folder will contain the treatment plan, all reports and a slot for our business card and clinic brochure.

I am looking for an original design and not just copy and paste my business card onto it. I would like to add an infographic on the inside page of the folder.
