-Query Set: (Internal Use Only) A Query Set is like a chapter in a book; they group together similar queries underneath a descriptive header.
-Query: A Query is a targeted question that asks a user for related inputs, or Values.
(a) Radio Group: For inputs in which only one value is correct. Limit of 5 values - for greater values, users should select “Drop Down Menu”. E.G., Yes/No
(b) Drop Down Menu: For inputs in which only one value is correct. No value limit.
(c) Checkboxes: For inputs in which none, one or more values could be selected simultaneously.
(d) Digit Field: For numerical inputs that may vary in size, and for which a precise measurement is needed. Users may set a range of entries to help correct input errors. E.G., 7897 mL
(e) Text: For inputs that must be written or typed.
(f) Scale: For inputs that fall within a limited range of values, and for which a sliding scale is ideal user experience.
(g) Upload: For Values best uploaded in original form.

WHAT: A customized and friendly digital report for the patient, which aggregates all data gathered via Surveys during his/her whole treatment. The goal is to summarize results and help the patient understand his/her treatment.
