'Maipenrai' is Thai for 'no worries, I'm fine, it's ok' etc, so something incorporating something classically Thai like a palm tree for example would be good. If possible, incorporating something surgery related as well would be nice, as long as it doesn't make the design look too fussy, eg a bone, instrument or somesuch, but not the Caduceus
