About us
We support complex people with learning disabilities and mental health issues to leave plan on how they want to live once they have left long stay hospitals. We work with them and their loved ones to make sure their future home and support is tailored to their needs instead of people having to fit into existing services. Our detailed planning helps people get out of hospitals - which are not great places to live. We help people and their families to take back choice and control in their lives.

Our information is aimed at a mixed audience as we work with; people with learning disabilities and other complex people, families as well as clinicians and professionals. We would like our logo to reflect the values of our organisation which is about people having meaningful lives outside of hospitals.
