Lifetime Health Diary
LifetimeHealthDiary.com is a patient-centered personal health record application. It enables patients to record their symptoms and wellness and share them with healthcare professionals, carers who can then collaborate to improve healthcare outcomes.
Lifetime Health Diary(tm) is a innovative online platform that integrates clinical data and background health data around the patient, engages patients in monitoring their own health, provides clinicians with a comprehensive, longitudinal history of a patient’s health background via “temporal correlation” (one summary page at a glance), and creates a comprehensive and holistic view of the patient.
It is a disruptive technology shifting the way health information is managed by wrapping it around the patient. It provides not only a better way of communication between patients and multiple caregivers but all the more has an ability to look at the patient in a holistic manner leading to much better health outcomes.
What a great logo (a real one, not just an encryption) would do is:
- Create brand identity
- Communicate with the users and other stakeholders
- Reflect our mission and vision (patient-centric app, creating mutual dialogues, improving health outcomes, etc)
- Highlight that LHD is an innovative company in healthcare industry, trustworthy and credible
- Sticks in the memory
- Associates with our products and company as we go further and start releasing other products under the same umbrella.

Target users: 1) patients (chronic disease, etc); 2) caregivers: social worker, nurse, physician, doctors, family members, etc. who may enter some/all info on behalf of the ones they care for; 3) pharmacists
We have a tagline: "you are your caregiver on the same page. Your page" bu that tagline can be reworded to reflect a better copy bu the meaning stays the same.
