Tell us a bit about who you are and the people you reach
I am opening a General Dental office that will be upscale and attractive for high end clients. I am looking for a logo that is mature (can be done by design or color scheme), sophisticated, sleek, modern, and I love logos that incorporate hidden dental images within a larger image (i.e. tooth as part of the lotus, or implant as part of the dental tooth). Target audience is high end cosmetic dental client, or those looking for modern dental procedures such as dental implants or veneers. Dental implants will be a large part of the practice. One option is to also transition from a hard angled 3D tooth (similar to enclosed pic) to a smooth surface (either 3d or 2d) to represent the process of something rough turning into something smooth. I would like the color of the tooth to have some hue of gold color transitioning to a different color. Placement of the font is important, as well as differentiation of words "Modern" vs "Dental" by use of different colored windows (see sample). PLEASE do not emphasize the "Lake Forest" in either the logo image, Lake Forest is a city but I do not want to draw any emphasis to the city name, more "Modern" "Dental" than the city part. Any image with a tree will automatically be declined.

Colors have a powerful impact, I really want part of the tooth to be gold transitioning to a different color, and also the presentation of the words and fonts are critical. If there can be some aspect of implants into the logo that would be a huge bonus, maybe incorporate the wavy threads somehow. (The pics below from Advanced Dentistry of Coral Springs and White are my favorite inspiration photos, as well as the spiral to represent implant threads)
