LEAP is a Hepatitis C Linkage to Care and Care Coordination program embedded in the Institute for Liver Medicine at the Icahn School of Medicine at Mount Sinai in New York City. We largely serve a vulnerable population of patients who are often hard to reach and many of whom have significant comorbidities and/or face complex psychosocial issues, such as substance use and homelessness. Initially, our team reaches out to patients who have been diagnosed with Hep C but have not been treated and, in some cases, don't know about their diagnosis. We provide initial education and link patients to medical providers who can treat their Hep C. We follow up until the patients make it to their first appointment. Once they do, we provide care coordination services to support them successfully through treatment. Care coordination is tailored to each patient and may include referrals to social and mental health services, coordination with pharmacies, and education on topics like liver health and avoiding reinfection. Our entire team provides patients with the kind of personal attention and support that is often missing in complex healthcare systems. We meet patients “where they’re at” to understand their unique situation and address any needs that might be preventing them from completing their 2-6-month Hep C treatment. In the process, we’ve seen patients gain new trust in the healthcare system and take control of their health in ways that will benefit them long after their Hep C is cured.
We are looking for a simple yet memorable modern design that reflects LEAP’s caring and empowering mission to patients, medical providers, and our funders. We prefer not to use literal images, such as a liver, unless they’re abstractly rendered and/or subtly integrated into the lettering. We are not wedded to a particular color scheme. However, we do not much like the red and yellow combination used for Hep C ribbons.
