Tell us a bit about who you are and the people you reach
The UICSL and its component programs have over 40 years of experience working with Salt Lake area AI/AN residents. The Sacred Paths Youth Services and Cedar Point Health Clinic programs both have experienced managers who administer federal and state funding for their programs as a matter of course. Sacred Paths, in particular, works extensively with the AI/AN youth population and has staff with extensive experience in tobacco and other substance abuse prevention programs. Cedar Point regularly holds activities and educational workshops that encourage various ways to live healthy. Additionally, the program includes a licensed clinical social worker and a clinical caseworker who do home visits, work with youth and adults who have been screened for suicidal ideation, and presents at functions in the community on historical trauma and its effects. During our first year of the project, we will recruit additional local agencies to cover service gaps that UICSL programs do not currently cover, including psychiatric assessment, medication prescription and maintenance, inpatient mental health and substance abuse treatment, and appropriate detox facilities for young people

Attached is our overall agency logo. Incorporate if possible. We would like the new logo around the medicine wheel concept while including some symbolism around "family" as we are a family clinic. A Cedar tree does not need to be the pinnacle
