Name, Email, Phone The Form won't need to have every field that's in the example web pages. We only need to Capture Zip Code, Name, Email, Phone. After capturing the info, the last page will redirect to a page that has insurance companies that are looking to convert that traffic into a form fill. Imaging can be either Photo or Cartoon, Drawing Image. No really dark colors, black, browns, dark greys etc. Lighter colors work better.

Form can be split or on 1st page. If split, 1st page should have Zipcode only. The attached image is also something I think works well with the landing page design. Not absolutely need, but feel free to incorporate into design.
