Story to tell in the infographic
We are looking for an infographic that clearly depicts the four-step process for chronic disease reporting and verification in Chicago Public Schools (CPS).
The 4 steps are -
1. Complete Student Medical Information form and return to school
2. Have doctor confirm student’s health condition
3. Provide school with student’s medication and related paperwork
4. Work with school to complete 504 Plan/IEP and/or Emergency Action Plan
This inforgraphic will be used as part of an educational intervention targeting CPS parents. The intervention is called CHAMP (Chronic Health Annual Medical Procedure). The chronic diseases we are focusing on include asthma, food allergy, and diabetes.
The chosen infographic will be included in PowerPoint presentations, on handouts, and on posters.

Elements to include in the infographic
Because asthma, food allergy, and diabetes are the chronic conditions of interest here, perhaps some child-friendly images around these conditions would be helpful.
While we understand that the use of words will be necessary, we would like this infographic to be very clearly and simply convey the 4 steps of chronic disease reporting and verification to people who may not be native English speakers and/or have a low literacy level. Inner-city parents are the primary target audience for this intervention, and we need to ensure that our materials are accessible to all.
CPS documents, 1-pagers, etc. are attached to provide designers with more information about our office and the system. In addition, it's probably best to stick to the Blue/Orange/White color scheme in order to be consistent with other materials that the Office of Student Health and Wellness is putting out.
