Child Health Investment Partnership (CHIP) of Roanoke Valley is an early childhood home visiting program. CHIP of Roanoke pairs pregnant moms, low-income children, birth to first-grade entry, and their guardians with a Community Health Nurse and Family Case Manager to help them access needed medical services, to assist with the management of chronic conditions and to provide developmental education, kindergarten preparation and regular child assessment and monitoring. These services work to help build parenting confidence and to provide support to families as they set and reach goals moving toward self-sufficiency. We also offer supplemental case management services that target specific needs: Prenatal Case Management, Health Care Coordination, Family Strengthening, Medically Needy & NICU Case Management, Oral Health Support, Asthma Case Management and Mental Health Counseling. This invitation will be used to attract potential donors

We need an invitation designed for our 30th Anniversary. Ideally, we are looking for a black backdrop with our logo colors incorporated. The goal of this Gala is to raise funds to support 30 children in honor of our 30th Anniversary.

We would like to have our 5 logo colors instead. (Do not use the gold or silver.) Please use: Cyan Blue, Magenta, Yellow, Grass Green and Red. Also LOVE the Merrill Lynch design. Event Details: Thursday September 13, 2018 Cocktail Hour: 6-7:30 PM at Black Dog Salvage Dinner: 7:30-10 PM at Mountain View Center Event is presented by United Healthcare Community Plan of Virginia. Use the logo attached. I have also attached the Sponsor Packet so that you can get an overall feel for the evening.
