What's your vision?
We are wanting to develop a medical score card that will be printed on plastic. The size must fit onto a ID badge size, roughly 3"x4". Frontside: It should cover the 5 areas of body exposure, which are Bariatric, Ambulation, Lateral, Vertical, and Reposition. These are the essential areas we train ergonomically for hospital staff when lifting/mobilizing patients. Also, under "each body area of exposure" we are wanting to add a barcode (for design purposes any generic barcode will do, just for appearances/place holder). This should all be inclusive to the front side of the badge. Attached is an idea that will give some direction but this scorecard design shouldn't look like this. Backside: Should look identical to the front but without barcodes and just empty boxes, so that we could physically place stickers or be able to place a sharpie mark within.
Attached is an example of the 5 areas of body exposure, located at the top (labeled in the columns). Looking at the attachment, the equipment on the left side is not needed. The "X's" and "check mark's" are essentially where the barcodes should go. Also attached is our company logo and company name Zero Lift Inc
