We work with physicians, hospitals and community organizations to improve health and the healthcare system. We largely focus on helping patients and providers manage chronic disease and ensure that all people have access to quality healthcare, regardless of their socio-economic status.

I need a rack card designed that physicians and nurses can hand out to patients who have a chronic disease. This is for adult patients of all ages (young adult to the elderly). I need to ensure that we are being diverse if any stock photos are used, as we are dedicated to quality care for all people, regardless of their socio-economic status. "Living Healthy" is a six week intensive course to teach patients and their care givers, how to live with chronic disease.I would like the tone of the card design to be hopeful. I am submitting another contest for a brochure for the physicians and I would like the two items to match. I have attached the copy for the brochure and the logo for Living Healthy (which is the name of the program)
