    The layout is similar to your current page.
    My idea would be to give the user only 2 options on the main page. Whatever they select would determine what the rest of the form would be. This would eliminate unnecessary questions for the user that selects 'No'.
    If you see my other submission, this is the layout if someone were to chose 'Yes'.
    I will submit the personal info page and the confirmation page shortly but would like to know your thoughts on the initial design.

    I am also curious as to how these will be tested prior to the winner being chosen.

    Feedback would be greatly appreciated.

    Regarding design #1

    Hi, thanks for the submission. I like the idea of limiting the number of initial questions to one y/n question, and shaping the subsequent pages accordingly.

    We will not test the designs in order to declare a winner. Our team will choose one or more designs that we will code and test.

    Regarding design #2

    As commented earlier: the yes/no idea is very interesting, especially when it brings down the number of questions on the second page. The overall lay out of this second page is quite similar to the current design. I wonder why you have decided to remove the image on this page?
