Regarding design #2
Hi thanks for the entry.
This is the right direction. The form doesn't need to be split up as much. It could have the fields above each other taking up 1/3 of the space, thats not to say it has to be though. I'll ask my collegue what he thinks when he's back.
The header might be a little big, I'm not a huge fan of the picture of the guy either. Happy families or pretty call centre girls would work well.
On a positive note, the colours are ok and it is easy to read.
